This position is responsible for conducting virtual or face-to-face assessments of members who may need skilled nurse assessment services. The incumbent completes all assessments using a skilled care acuity tool and provides a recommendation for services based on members health care needs. Members who are assessed reside in the community or are ready to be discharged from a facility.
The incumbent may also serve as mentor or preceptor for new staff, assisting in the formal training of assessors, complete quality assurance audits, inter-rater-reliability reviews, and may be involved in process improvement initiatives.
Job Title: Assessor Nurse (RN) **Bilingual in Spanish highly preferable**
Locations: Hybrid/Field and Remote opportunities depending on location
- Iowa, Oklahoma & Idaho (100% remote) ***must reside in one of those states
- Colorado (hybrid, in the field) ***willing to travel around Western Colorado/Grand Junction, CO going into patients homes
Non-negotiables:
- BSN is preferable but open to ASN as well
- Bilingual/fluent in Spanish preferable
- Must have a Colorado license or Compact RN License
- Must have 3 years of nursing experience (not including clinicals) in the last 10 years
- Must have experience with IDD population
Hours: 9am-5pm MST **must be somewhat flexible if a patient needs them to meet at a different time
Pay:
75-85k for Colorado
70-80k for Iowa, Oklahoma, Idaho
Essential Functions:
- Performs face-to-face and/or virtual assessments of persons with disabilities. Collaborates with health care providers and ancillary service providers to create or validate the appropriate treatment plan.
- Documents assessment results for the individual member status or negotiates service plan as needed.
- Completes quality assurance competency reviews and other tasks/projects as assigned such as new assessor training.
- Provides ongoing education. May communicate with all health care providers to ensure appropriateness of care.
- Performs miscellaneous duties as assigned.
Requirements:
- Experience working with individuals with disabilities, individuals with intellectual and developmental disabilities (IDD).
- Experience working with Pediatric and Geriatric populations with various disabilities, and individuals with complex health needs preferred.
- Home health, case management or virtual assessment experience
- Current and unrestricted RN licensure in Colorado or a compact nursing license preferred.
- Bilingual or Spanish speaking highly preferable.
Contract Details:
This is a long term temporary position that will be expected to be at least 6 months but could extend based on the backlog and needs of the program.
