Job Title: Bilingual Case Manager - Home Study & Post Release Services (HS/PRS)

Reports To: Lead Bilingual Case Manager – HS/PRS

Department: Programs – El Caminos

Location: Gainesville, GA

FLSA: Full-time Exempt

Job Summary:

The HS/PRS Bilingual Case Manager provides culturally competent assessment and case management services for recently immigrated unaccompanied children and their U.S. sponsors. This role involves assessing the safety and suitability of potential sponsor homes and providing post-release services to support the youth’s community integration and family reunification. The Case Manager follows Wellroot Family Services' trauma-responsive model, offering comprehensive services to youth and caregivers to improve communication, provide psychoeducation, and connect them to community resources. Wellroot collaborates with the Board of Child Care to serve unaccompanied children across North Georgia and beyond.

Responsibilities:

Home Study Services:

  • Model and adhere to Wellroot core values
  • Conduct home studies via home visits, interviews, phone calls, and background investigations to assess potential sponsors
  • Produce comprehensive reports on home study findings within 10 days of assignment
  • Provide psychoeducation to youth and sponsors, identifying support needs for successful reunification

Post Release Services:

  • Conduct home visits and phone contacts to assess safety and connect youth to community services
  • Perform ongoing case management, making referrals in various domains such as education, health, mental health, legal, and more
  • Develop and update youth and family care plans with measurable outcomes
  • Provide crisis intervention services as needed
  • Facilitate case management meetings with youth, sponsors, and external providers
  • Maintain regular contact with youth and sponsors, adhering to performance metrics

Overall Responsibilities:

  • Serve as the primary contact for youth, families, sponsors, and external agencies
  • Travel within a 200-mile radius or beyond as needed
  • Participate in community efforts for immigrant and refugee outreach and education
  • Complete required documentation and reports within set timelines
  • Coordinate with Wellroot staff and community resources to meet youths’ service needs
  • Maintain effective relationships with all stakeholders
  • Identify supervision and training needs
  • Express ideas clearly in both oral and written communication
  • Engage youth and families positively

Non-Essential Functions:

Work Environment/Remote Office Requirements:

This role requires operating in both professional office and remote work settings. A fast, reliable internet service (15 Mbps minimum download speed) is needed. The remote workspace must support phone and video calls, maintain confidentiality, and store equipment securely. Wellroot provides a laptop and cell phone; the employee is responsible for other remote office equipment and costs.

Position Type/Expected Hours of Work:

This is a full-time position with a flexible schedule based on youth and family availability. It may require long hours and weekend work and is eligible for a flex schedule.

Travel:

Travel is required, including local, out-of-area, and overnight trips to serve the home communities of youth and families.

Requirements:

  • Bachelor’s degree in behavioral sciences, human services, or social services fields
  • Bilingual in English and Spanish (oral and written)

Preferred Education and Experience:

  • Master’s Degree in Social Work and 1-year experience in direct services to children/youth and families
  • SAFE Home Study certification or similar
  • Two years of experience with immigrant and/or refugee populations
  • Child welfare and/or ORR case management experience
  • Proficiency in languages other than English and Spanish

Additional Eligibility Qualifications:

  • Proficient with Microsoft Windows 10 and Office 365 (Word, Excel, PowerPoint, Outlook)
  • Effective time management, planning, communication, and interpersonal skills
  • Problem-solving and independent work abilities
  • Maintain a valid driver’s license and operate a motor vehicle
  • Pass CPR/First aid certification, CPS/Adam Walsh, Tuberculosis Screening, work-readiness physical, Federal and State Fingerprinting Background Check, and drug screen
