Job Title: Senior LTSS Service Care Manager (RN)
Duration: 06 months Contract (Potential to Extend)
Location: Remote – Midland/Odessa, TX or surrounding area.
Shift Type: 8-5 Mon-Fri
Job Duties:
• Service Coordinators generally work standard business hours, Monday through Friday, with flexibility needed to meet members and family schedules. Travel is a large part of the role, so days may include both in-office (Remote) documentation time and field visits. Overtime is not expected as a routine, but occasional extended hours may be necessary to meet state deadlines or accommodate urgent member needs.
• The day is typically divided between member-facing activities and documentation. This includes conducting health assessments, service plan development, follow-up contacts, provider coordination, and charting in the system. Case prioritization is driven by state timelines and acuity of member needs. Service Coordinators usually balance a mix of scheduled assessments, phone calls with families/providers, and required documentation tasks.
• Although much of the work is done independently in the field or remotely, Service Coordinators stay closely connected to their team. Interaction occurs daily through case discussions, check-ins with supervisors, and collaboration with other team members. Strong communication is essential since many cases require coordination across multiple roles.
• This role is field based with a combination of travel, remote work/office time. Service Coordinators must be comfortable entering member homes, engaging directly with families, and working with providers in the community. When not in the field, coordinators work from their home office to complete documentation and team meetings. The environment is dynamic, member-driven, and requires both independence and teamwork.
Must Have:
• Pediatric Nursing Experience (2–3 years minimum) – direct clinical background in pediatrics is essential for understanding member needs and providing appropriate care coordination.
• Case Management & Care Coordination – ability to complete assessments, develop service plans, and navigate regulatory timelines (experience in managed care or Medicaid is highly preferred).
• Proficiency in Microsoft Office (Excel, Word, Outlook) – required for documentation, reporting, and communication within the health plan’s systems.
• Strong Organizational Skills – ability to manage a large caseload, track timelines, and keep documentation accurate and timely.
• Problem-Solving – skill in addressing barriers to care, adapting to unique member situations, and finding practical solutions in real time.
• Effective Communication – clear, professional interaction with families, providers, and team members, especially in sensitive or high-stress situations.
Nice to have:
• Case management experience is preferred and considered a strong advantage.
• Prior experience working with Medicaid populations or in home/community-based settings is beneficial.
Disqualifiers:
• Unwillingness or inability to travel for home visits.
• Inability to work a consistent Monday–Friday schedule.
• Lack of pediatric experience.
• Poor organizational skills or inability to manage a caseload independently.
• Inability to use Microsoft Office applications, including Excel, Word, and Outlook, for documentation and reporting.
