Title: Care Manager - Licensed Social Worker LSW

Job Type: Permanent / Direct Hire

Setting: Home Health (hybrid role after 30-day training period)

Location: Blue Ash, OH

Schedule: Full-time, Mon – Fri, 8a – 4:30p

Pay: $60,000-65,000/year, based on experience plus full benefits

JOB DESCRIPTION:

The Care Manager LSW will provide intake, assessment, and care management services to clients, which includes ongoing assessment for program eligibility, monitoring all aspects of care and client services (social and medical), and ensuring needs of clients and caregivers are met through a collaborative process.

REQUIREMENTS:

• OH Licensed Social Worker (LSW) in good standing
• Required experience: previous work in home healthcare, hospice, and/or case management, minimum 1 year to be considered
• Prior experience with elderly populations or people with disability preferred
• Valid driver’s license with good record and reliable transportation
