Job Description

The 4th Judicial District, Schenectady County Treatment Court, is seeking a dedicated and compassionate individual for the position of Case Manager. This role involves providing comprehensive case management and monitoring services to participants in the Treatment Court program. The Case Manager will work closely with participants to ensure they receive appropriate services, support, and guidance throughout their involvement in the program.

Key Responsibilities:

• Conduct interviews with participants to assess their needs for services and develop personalized case plans.
• Refer participants to psychological, counseling, and other support services as necessary.
• Conduct psycho-social evaluations and prepare detailed reports for the court.
• Monitor participant compliance with program requirements and recommend appropriate sanctions or interventions.
• Make preliminary case recommendations to the court regarding participant progress and service needs.
• Assess participants’ progress in treatment and other services, identifying any changes or additional needs.
• Assist in preparing clients for discharge to the community, ensuring they have the resources and support needed for successful reintegration.

Qualifications:

Bachelors Degree from an accredited college or university and one (1) year of satisfactory full-time experience in social casework with a recognized public or private agency adhering to accepted professional standards;

or

Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and two (2) years of satisfactory full-time experience in social casework with a recognized public or private agency adhering to accepted professional standards;

or

An equivalent combination of education and experience.
