We are seeking a Nurse Practitioner specializing in Wound Care to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients in skilled nursing and assisted living settings. This full-time position requires the NP to travel to Middlesex and Monmouth Counties and surrounding areas to perform wound care in SNFs and ALFs.

What started with a singular vision to assist nursing home patients has now expanded into a full-fledged wound care conglomerate providing care to over 2000 patients per week. We pride ourselves in the fact that we travel to the patient and not the other way around. Our clinicians are providing service to patients in long term care and assisted living facilities. Whether caring for geriatric patients, post-surgical patients, or those with complex health needs, we are committed to providing the most holistic, quality care possible in every case.
Responsibilities:
Manage wound care at skilled nursing and assisted living facilities
Travel to multiple skilled nursing and ALFs
Diagnose and establish patient treatment plans
Assess, monitor and report changes in patient symptoms
Communicate with collaborating physicians or specialists regarding wound care
Facilitate referrals to other healthcare professionals and medical facilities
Maintain accurate patient medical records
Provide advice and emotional support to patients and their family members
Qualifications:
Previous experience in nursing field
Familiarity with medical software and equipment
Ability to build rapport with patients
Strong problem solving and critical thinking skills
Ability to thrive in a fast-paced environment
Knowledge and experience with wound and ostomy care
Education and Experience:
Master of Science in Nursing
Must be boarded in NJ
BOARD ELIGIBLE IS NOT ACCEPTED
DEA license
Excellent customer service skills

NO night or weekend calls, great staff to work with!
